Physical Therapy Services

Our team will begin physical therapy services with an evaluation to assess current levels of functioning and help us to get to the root cause of the patient's condition. Following the evaluation, the physical therapist will collaborate with the patient to create a tailored plan of care that will help the patient get back to their daily activities. Once physical therapy begins, direct therapy and home protocols will be provided to lessen pain and to restore, improve, or maintain strength, fitness levels, endurance, flexibility, and mobility.

Physical Therapy Evaluations

Elite Therapy and Wellness recommends a physical therapy evaluation for individuals experiencing any of the following:

  • Arm, shoulder, leg, hip, spine injuries

  • Chronic conditions like arthritis and MS

  • Physical rehabilitation after a stroke or TBI

  • Declining strength, mobility, flexibility, or coordination

  • Recovery after a sprain, strain, or fracture

  • Shortness of breath, or dyspnea, with minimal movement

  • Spinal cord surgery or injury rehabilitation

  • Total joint arthroplasties

  • Neurological conditions such as ALS or Parkinson's

  • Loss of balance or vertigo

  • Difficulty walking

  • Fall prevention

  • Need to improve or maintain physical wellness

Person-Centered Therapy

Good-Faith Estimate

The No Surprises Act

At Elite, when you schedule a speech, physical, or occupational therapy service, we provide a Good-Faith Estimate of the cost of the service.

What you should expect from your health care provider:

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item.

Dispute conditions:

In short, if you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

What do you need to do?

Make sure to save a copy or, per the CMS guidelines, take a picture of the Good Faith Estimate that was provided to you.
